3. SMS / Text Message Communications

If you opt in to SMS communications by selecting the optional consent checkbox on our booking form, you agree to receive SMS text messages from Equinox Integrative Health. Messages may include appointment confirmations, reminders, scheduling updates, and feedback requests. Message frequency varies. Standard message and data rates may apply. Reply STOP to opt out at any time. Reply HELP for assistance. SMS is used for scheduling purposes only — never for medical advice or PHI.

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

4. HIPAA Notice

Dr. Allison McClelland, M.D. is subject to the Health Insurance Portability and Accountability Act (HIPAA). Your protected health information (PHI) is handled per our Notice of Privacy Practices, available upon request. We implement appropriate administrative, physical, and technical safeguards to protect your PHI.

For non-PHI personal information, we implement reasonable technical and organizational safeguards including secure data transmission (SSL/TLS encryption), restricted access controls, and regular security reviews to protect your personal information against unauthorized access, loss, or misuse. 5. Cookies and Tracking

Our website uses Google Fonts, which may log your IP address as part of font delivery. We do not use advertising cookies or third-party tracking pixels.

• Scheduling: Appointment data is processed by our booking platform.
• Payment Processing: Handled by Stripe, Inc. We do not store credit card data.

Patient records are retained per Arizona state law — minimum 6 years from date of service, or 3 years after a minor patient reaches age 18, whichever is longer.
